Effect of iron deficiency without anaemia on days alive and out of hospital in patients undergoing valvular heart surgery

Comprehensive evidence regarding the treatment of non-anaemic iron deficiency in patients undergoing valvular heart surgery is lacking. This study aimed to investigate the association between non-anaemic iron deficiency and postoperative outcomes in these patients. We retrospectively analysed 321 patients of which 180 (56%) had iron deficiency (defined as serum ferritin < 100 ng.ml^-1 or < 300 ng.ml^-1 with transferrin saturation < 20%). While the iron-deficient group had lower pre-operative haemoglobin levels than the non-iron deficient group (median (IQR [range]) 134 (127–141 [120–172]) g.l^-1, 143 (133–150 [120–179]) g.l^-1, p = 0.001), there was no between-group difference in allogeneic red blood cell transfusion. Median (IQR [range]) days alive and out of hospital at postoperative day 90 was 1 day shorter in the iron-deficient group (80 (77–82 [9–85]) days vs. 81 (79–83 [0–85]) days, p = 0.026). In multivariable analysis, only cardiopulmonary bypass duration (p = 0.032) and intra-operative allogeneic red blood cell transfusion (p = 0.011) were significantly associated with reduced days alive and out of hospital at postoperative day 90. Iron deficiency did not exert any adverse influence on secondary outcomes except length of hospital stay. Our findings indicate that non-anaemic iron deficiency alone is not associated with adverse effects in patients undergoing valvular heart surgery when it does not translate into an increased risk of allogeneic transfusion.     

左、右半结肠癌肝转移患者射频消融治疗疗效及生存的对比分析

目的:探讨原发部位分别位于左、右半结肠的结肠癌肝转移患者行超声引导下肝转移瘤射频消融术后的生存差异及预后影响因素分析。方法:回顾性分析2010年1月至2015年12月于我科行射频消融治疗的结肠癌肝转移患者100例，其中左半结肠癌62例，右半结肠癌38例，分析原发部位不同的结肠癌肝转移患者在进行超声引导下射频消融治疗后的生存情况。结果:左半结肠癌肝转移患者射频消融术后1、3、5年总体生存率及中位生存期均高于右半结肠癌肝转移患者（88.71% vs 73.68%、62.90% vs 30.02%、49.72% vs 26.33%、55个月 vs 23个月），且二者生存曲线差异具有统计学意义（P＜0.05）。经Log-rank检验及COX多因素分析，左、右半结肠癌肝转移患者射频消融治疗术后的生存情况存在差异。此外，原发灶淋巴结转移情况、患者术前CA199水平及原发灶分化程度是患者射频消融治疗术后生存情况的影响因素。结论:左半结肠癌肝转移患者射频消融术后整体生存率明显高于右半结肠癌肝转移患者，左半结肠癌与右半结肠癌肝转移患者在射频消融治疗术后生存状况存在明显差异。     

The Added Value of SARC-F to Prescreening Using FRAX for Hip Fracture Prevention in Older Community Adults

To examine the potential added value of a simple 5-item questionnaire for sarcopenia screening (SARC-F) to the Fracture Risk Assessment Tool (FRAX) for hip fracture risk prediction, in order to identify at-risk older adults for screening with dual-energy x-ray absorptiometry (DXA).

基于5G技术的远程会诊系统应用

介绍5G网络技术特点，以江苏省中医院的互联网医院建设实践为例，阐述现有网络5G改造以及基于5G的远程会诊系统构建与应用，指出该系统能够提高远程教学及多学科联合诊疗的效率和准确性。     

千金苇茎汤对香烟烟雾暴露模型大鼠肺组织Th17/Treg分化及其相关细胞因子表达的影响

目的:观察千金苇茎汤对香烟烟雾暴露模型大鼠肺组织辅助性T细胞17(Th17)/调节性T细胞(Treg)分化及其相关细胞因子表达的影响。方法:雄性大鼠60只,随机分为正常组,烟雾模型组,乙酰半胱氨酸组、千金苇茎汤高、中、低剂量组,共6组,每组10只。香烟烟雾造模30 d后,药物分别干预30 d,后处死各组大鼠并取组织;实时荧光定量聚合酶链式反应(Realtime PCR)检测维甲酸相关孤儿受体(ROR-γt),叉头状/翼状螺旋转录因子3(Foxp3)mRNA表达,酶联免疫吸附测定(ELISA)检测肺组织白细胞介素-17(IL~(-1)7),IL-6,IL~(-1)0,转化生长因子-β_1(TGF-β_1)含量,苏木素-伊红(HE)染色切片观察肺组织病理,流式细胞术检测Treg(Foxp3~+CD25~+)比例,Th17(CD4~+IL~(-1)7~+)比例,Treg/Th17。结果:与正常组比较,模型组ROR-γt mRNA表达显著升高(P0.01);与模型组比较,千金苇茎汤高、中、低剂量组干预后都能够降低其表达(P0.05,P0.01);与正常组比较,模型组Foxp3 mRNA表达明显降低(P0.05),与模型组比较,千金苇茎汤中低剂量组干预后表达明显升高(P0.05)。与正常组比较,模型组IL~(-1)7,IL-6含量明显升高,IL~(-1)0,TGF-β1含量明显降低(P0.05,P0.01);与模型组比较,千金苇茎汤高、中、低剂量组干预后能够降低肺组织IL~(-1)7,IL-6含量,同时升高IL~(-1)0的含量(P0.05),与Real-time PCR检测的结果趋势一致。流式细胞术检测显示,千金苇茎汤中、低剂量组干预后Th17(CD4~+IL~(-1)7~+)比例降低,Treg/Th17增加(P0.05,P0.01)。结论:千金苇茎汤能调节Treg/Th17,使Th17/Treg达到平衡状态。
